One percent of people with complex care needs account for 20% of healthcare expenditures in the United States, creating enormous challenges for physicians. These patients often have multiple chronic medical, mental or behavioral health challenges, and face food insecurity, homelessness, and lack social support or transportation. How do we evolve from providing episodic care to delivering exceptional care to these individuals? In this session, learn about the:
