Jay Rajda

Dr. Jay Rajda is the Chief Clinical Transformation Officer and Senior Medical Director with Aetna Health and Clinical Services where he is responsible for clinical innovation, and the development of innovative, community based personalized care models. Previously, he worked at Aetna's Innovation Labs, where he worked on identifying, validating, and deploying innovative clinical and technology ideas a few years ahead of the market. Prior to that, he was Medical Director at ActiveHealth Management, where he was responsible for the development of clinical analytics, clinical decision support, performance improvement and care management programs. He also provided clinical expertise in implementing value-based care delivery and population health management solutions to several client organizations including plan sponsors, provider organizations and ACOs. Board certified in Internal Medicine and a Fellow of the American College of Physicians, Dr. Rajda has served as Clinical Assistant Professor of Medicine at the University of Rochester. He obtained his medical degree from the University of Mumbai, and an MBA from the University of Rochester’s Simon School, graduating first in his class. He is an active member of the New York City health innovation community.
